Computer Request Form

The computer request form is required to obtain a workstation for new staff and faculty, existing staff and faculty, student staff, classrooms and labs.  Please see the requirements of the necessary workstation before proceeding with the form.

If you have a machine that is up for refresh, you can view available refresh models here.

Click HERE for the computer request form, this form should be submitted to the IMT Support Desk five business days prior to the employee start date to ensure timely delivery of equipment.

PLEASE NOTE: If these computers fall within the Departmental Purchase of Computers policy and our Student Staff Computer philosophy, the following will apply:

  • Computers will include standard software and full support for four years
  • Computers purchased under this policy are not eligible for refresh
  • Laptop computers are not available for student employees 

Computer for Classrooms and Labs

  • These computers must have a justification provided and approved by IMT
  • Must have Dean or Vice President level approval and signature
  • Upon approval, the requesting department must fund the initial purchase of the computer
  • Computers purchased for labs will be in future refreshes

Refreshing a computer before the scheduled time...

  • Must be requested through the Office of the CIO
  • Must have Dean or Vice President level approval and signature
  • Must provide a departmental account number for the computer to be charged against
  • The computer being refreshed will be returned to IMT and cannot be kept by the
    department for other computer needs
